Description of work
In this entry-level position, the employee will work under the close supervision of the supervisor to develop the necessary care management skills. This position will serve as the primary information and referral person for the agency and also manage a reduced caseload. This requires answering all calls and assisting walk-in clients requesting information about agency services or referrals for service. The Aging Care Manager will also work closely with other social service agencies, hospitals, clinics, and community resources in planning to meet the needs of clients and assist the clients in utilizing these resources. An important aspect of the work is the employment of care management skills in obtaining essential information from consumers and families via required assessments to determine what needs they have and determine the best avenue to meet them. Prior to being able to complete the required assessments, the Care Manager will need to be credentialed with the State. Work is performed in accordance with established regulations, policies, and procedures, but employees are expected to exercise initiative and independent judgement in discharging their duties.
Minimum Requirements:
- Must meet PA residency requirements.
- A Bachelor's Degree which includes or is supplemented by 12 college-level credit hours in sociology, social welfare, psychology, gerontology or other related social sciences; or three and one-half years of experience as an Aging Case Aide and 12 college-level credit hours in sociology social welfare, psychology gerontology or other related social sciences; or any equivalent combination of experience and training which includes 12 college-level credit hours in sociology social welfare, psychology, gerontology or other related social sciences.
- Must be able to perform essential job functions.
